Privacy Policy

In an effort to protect your privacy and ensure the confidentiality of your personal information, we have put in place a policy related to the collection, use and protection of this personal data.

Responsibility

The Clinique Dentaire Dac is responsible for all personal information under its control.

Ms. Manon Ruel has been appointed as the person responsible for the protection of your personal information according to the regulations stipulated in the Loi 25. In addition, other members of our team will be responsible for the collection, and processing of your personal information. Our team is trained to understand the importance of all data collected and how to process it securely.

We are responsible for all personal information in our possession, including information transferred to us by a third party for the purpose of dental treatment. Through contracted providers, we use a high level of protection for the processing of all personal data as well as data transmitted to and by third parties.

Collection

We gather personal information directly from you or from other authorized people who obtained your consent for the transmission of that information. The type of personal information provided to us includes your name, date of birth, telephone number, home address, email address, and medical history. This information is collected to provide you with proper dental health care.

Usage

We identify the need to collect your personal information before or at the time your personal information is required.

We collect, use, and disclose your personal information for the following reasons:

  • To plan a dental treatment
  • To follow up on a dental treatment, and dental billing
  • To obtain a payment for services rendered (from you, an insurance carrier, or other)
  • To manage your patient account
  • To train and educate our staff
  • To pursue improving our quality
  • To pursue risk management activities
  • To compile statistics
  • To pursue research to improve our services
  • To conform to regulations and legal obligations
  • For other uses allowed by law

We will collect, use, and disclose only personal information that is necessary to achieve our goal. When you sign the Patient Consent Form, we consider that you accept our Privacy Policy relative to the collection, usage, and disclosure of your personal information.

We will also obtain your consent before your personal data is used or disclosed. If you are uncertain of the objective or reason for which we are collecting your information, we will be happy to answer any questions you may have.

Consent

Your personal information will only be collected, utilized, and disclosed with your explicit consent or with the full knowledge for the use of this information; exceptionally in emergency cases or other occasions allowed for by law.
If your personal information has to be used for a situation that has not been previously stated, your consent will be required.

The exception, a parent or legal guardian can consent in the name of a person less than 14 years of age.

You can withdraw your consent at any time, subject to legal or contractual provisions, and be notified in a reasonable delay. Please communicate with Ms. Manon Ruel to maintain or withdraw your consent to allow us to collect, use, and disclose your personal information.

Limited Data

We limit the collection of personal information to the quality and quantity and type of data necessary for the sole purpose that we have identified. All the information is collected properly and legally.

Limiting the use, disclosure, and safekeeping

Your personal information will not be used or disclosed except for the purpose for which the data was collected; with the exception of your explicit consent or if it is allowed within the scope of the law.

Personal data can be treated, utilized, stored, or consulted with in other countries and can be subject to the laws of these countries. We will use all contractual means of providing a high level of protection for all personal information that is consulted and treated by third parties.

Your personal information will be kept for the necessary amount of time required to fulfill the mandate for which the data was collected. We limit the use of your personal data; and these are not disclosed for use other than the reason they were collected in the first place; except if we obtain your consent or if required by law.

Once your personal information is no longer required to fulfill the reasons we have identified, it will be destroyed, erased, or modified to conserve your anonymity.

Updating

We will do our best to keep your personal information precisely, completely, and up to date in as far as it is needed. We will do our best to keep your information updated with any new data.

If your personal information changes or if you think the information that we have on file is inaccurate, please communicate with us to advise us of any changes to be made to your file.

We keep all the information safely in order to prevent the loss, theft, unauthorized use, changes, copies, or disclosure. When we no longer need your personal information, we take the necessary precautions to destroy the data, much like when we store it.

Depending on the format where your information is collected, security measures could include physical protections – such as a locked filing cabinet, limited access to such a locked cabinet, office, or files; limiting personnel access to such areas, technologies, passwords, and encrypted files.
