How to prepare?

Health questionnaire
Upon your arrival, you will be greeted by one of our experienced receptionist and will be asked to complete a confidential health questionnaire. You may print a copy directly from our website under the section “online medical questionnaire”, fill it and bring it with you, or we will provide one upon your arrival at the clinic.
Please arrive a few minutes before your scheduled appointment. If you are taking any medications we ask that you bring a list of them with you.
To begin
Depending on the nature of your first visit, you will be escorted to our consultation rooms either by our hygienist or dental assistant. We will review with you the information completed in your health questionnaire in the privacy of your treatment room. At anytime, please feel free to communicate to us any expectations, concerns or questions that you may have.
Complete exam
We will proceed with a thorough extra and intra oral examination, after which we will go over our findings with you. If further treatment is required, we will explain in detail your options concerning the work to be done, as well as discuss necessary appointments and fees.
Cleaning and scaling
If your first visit is scheduled with a hygienist, she will proceed with the polishing and scaling of your teeth and give you oral hygiene instructions.
Follow up appointments
The time and number of visits required for the indicated treatments will be scheduled according to your specific dental needs as well as your availability.
Payment
Each visit is payable at the time of treatment. For your convenience, we accept cash, cheque, credit card or debit card.
Should you require a more flexible payment plan, we will be happy to provide you with the names of specialized dental financing companies.
Dental insurance
If you are covered by a dental plan, we will gladly submit your claim electronically to accelerate your reimbursement. When you arrive, please share with us all pertinent information related to your dental plan. If you don’t have the necessary information or if your insurance company does not accept electronic transmission, we will provide you with a printed dental form that you can submit to your insurer.
Our fees are the same for all of our patients, insured or not. Required or prescribed treatments are based solely on the optimum dental needs of our patients and not on the terms of insurance dental plans. If needed, we will gladly submit a predetermination (including all necessary documentation and information), in order for your insurer to evaluate the reimbursement you are entitled to.
